How to register

To register as a patient with West Timperley Medical Centre, you can request a new patient information pack at reception. Alternatively, you can download the appropriate Patient Pack below and a GSM1 Form and fill them out and send them in via email or print them off and hand them in at reception

Please ensure that you live within the practice boundary before registering. Unfortunately we are unable to accept requests from patients residing outside this area.

We need BOTH the New patient pack and the GMS1 form to be completed in order to process your registration.

Patient Access Forms (online account)

We ask that patients set up an account with Patient Access so that they can manage their own prescription requests and view test results/medical records online.

Please complete this form to set up Patient Access. The forms must be returned to us with a copy of your ID

If you want to set up Patient Access for your child who is under the age of 11. Please complete this form – this needs to be returned to the Surgery with the ID documents stated on the form.

Temporary patients

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient please contact the local practice you wish to use. For example, if you are studying at university and were previously registered with us, you are also seen as a temporary resident. If you are not registered with West Timperley Medical Centre but are a temporary resident within the practice boundary you will have to complete a temporary services form.
